PLEASE READ THIS & CHECK YOUR PROOF CAREFULLY
Every care has been taken to follow your instructions, but the final responsibility for the accuracy of this proof rests with YOU the client. Once you have submitted this form you have given approval for the production of your final artwork – in the case where there are errors on the final images which were not alerted to on the attached proof – it is solely the client’s responsibility.
Please check the proof thoroughly as liability by FIVE POINT AGENCY is limited to corrections only as indicated on the proof.

CHECKLIST
Refer to this checklist to make sure your proofs are error-free. Please note, that we cannot proceed to production without confirmation that these items are correct. *
- Correct fonts were used / All text is spaced properly / All text is formatted and aligned correctly
- All images are correct / Colours are correct - All important information is included content, who, what, where, when, why
- All wording/phrasing is typed correctly
- All spelling and grammar is correct
- Spelling of any names & places is correct
- All contact numbers are correct
- Physical, email and web addresses are correct
- Stated dimensions of images are correct

COLOUR REPRODUCTION WAIVER ( digital samples )
Since all computer monitors and devices display colour differently, we cannot guarantee print colours will match colours viewed on screen or when printed. For these reasons, we do not accept returns based on colour nor do we accept returns for approved proof errors. * I confirm that I have read and understood the information above and I understand that no liability will be attached to FIVE POINT AGENCY for any colour inconsistencies, perceived or otherwise, between my supplied files and the final printed product.
